The purpose of your visit will be to establish a bona fide doctor-patient relationship and to review your medical history & records. A HIPAA medical records file will be established at your initial visit.

Required information to bring with you:

FAILURE TO BRING ALL OF THE FOLLOWING WILL RESULT IN THE INABILITY TO FULFILL THE STATE OF MICHIGAN REQUIREMENTS FOR YOUR CERTIFICATION.

  • Picture ID, you must be a Michigan Resident
  • Medical Records documenting your Qualifying Condition(s)

 

 

 Payment at time of visit:

 

ALL NEW PATIENTS:

  • New Card, Renewal, or Annual Visit office fee–$115 cash or $125 debit/credit card
  • Compassionate office fees for new patients who are:
  • Veterans–$100  Thank you for your service!!
  • (please provide proof of service)
  • SSI or SSD recipients $100

(please provide documentation)

 

ESTABLISHED PATIENTS:

Renewal and Annual Follow Up visit–$75.00 ($5.00 cash discount) or $80.00 debit/credit card

State of Michigan MMMP Application fees:

  • Patient Application fee $40.00 (New Card or Renewal)
  • Great news……there is no longer a Caregiver fee!
  • Application and Caregiver fees to be enclosed with your application.

 

When you submit your application online at:  michigan.gov/bmronline
the State of Michigan fee of 40.00 is paid with a debit/credit card or online check.  If you have a CAREGIVER your application MUST be submitted by postal mail with a personal check, cashiers check or money order made out to:  The State of Michigan-MMMP   You can find the paper application and instructions for Patient/Caregiver here:

 

 

State of Michigan – MMMP

As a courtesy, if you bring the State fee with you in cash, we will write the check for you from our account, enclose the check with your application and mail it the same day.  There will be nothing further for you to do, except wait for your card to arrive in the mail!

The $60 fee is paid once every two years to maintain your MMMP Cardholder status.  Once issued, your card will be good for two years.
